  • Our facility meets NC building code standards for residential senior living, facility is ADA compliant, security system, life safety plan, and meets UL standards.

  • 1. Initial Assessment

    Before moving into Assurance Family Care Home, all potential residents must complete a brief assessment to confirm that their needs can be safely met. This assessment, lasting approximately 10-20 minutes, will be conducted by the Community’s Resident Care Director, who is a licensed nurse. The assessment includes a thorough review of the prospective resident’s physical and mental needs, medical history, and current medications.

    The assessment can be completed during your admissions meeting at the Community or scheduled for a separate time.

    2. Medical Examination FL-2 Form

    North Carolina mandates that each prospective resident must have a physician-signed FL-2 form completed within 90 days prior to admission to any assisted living or adult care home. This form must include the physician’s recommendation for residential assisted living as the appropriate level of care for the individual. Nicci will assist with any questions regarding this form and can coordinate with your primary care provider to ensure it is completed correctly.

    3. Community Fee

    Assurance Family Care Home requires a one-time, non-refundable $5,000 Community Fee for new residents. This fee is not a security deposit and covers the Community’s costs related to your admission, room preparation, orientation, and initial assessment, as well as general costs associated with the upkeep of the Community. 
     

    This fee must be paid before or at the time of the Residency Agreement execution.

    4. Residency Agreement

    The final step to becoming a resident of Assurance Family Care Home is signing the Residency Agreement. This contract outlines all the terms and conditions of your residency. Both you and your appointed representative will need to review and sign the Agreement. Nicci will ensure that all aspects of the Agreement are explained thoroughly before execution. The Agreement can be signed either in person or electronically.
